Crohn's Disease Symptoms: Warning Signs Adults Miss for Years
Crohn's disease does not announce itself. It creeps. Diarrhea for months, cramping that comes and goes, a few unplanned pounds off the scale, a fatigue nobody can pin down. Most patients spend years in that fog before anyone writes down the actual diagnosis. A prospective study in Inflammatory Bowel Diseases put the median diagnostic delay for Crohn's at around 9 months, with a long tail of patients waiting 5 years or more.
About 0.5% of U.S. adults have been diagnosed with Crohn's disease, and the age-adjusted prevalence has been climbing roughly 3.4% per year since the early 2000s. This piece covers what Crohn's actually does to the bowel, the symptoms that show up first, the extraintestinal signs that get missed, and the point at which "IBS-ish" chronic gut complaints deserve a real workup.

What Crohn's Disease Is Doing
Crohn's is one of two major inflammatory bowel diseases. The other is ulcerative colitis. In Crohn's, the immune system attacks the intestinal wall, triggering chronic inflammation that reaches through all layers of the bowel - mucosa, submucosa, muscle, and outer serosa. That "transmural" pattern is what separates it from ulcerative colitis, which stays in the innermost lining and is limited to the colon and rectum.
Two other features make Crohn's mechanically different. First, it can pop up anywhere from the mouth to the anus, but the classic site is the terminal ileum where the small intestine meets the large intestine. Second, the inflammation is "skip lesion" - patches of angry, ulcerated bowel with completely normal-looking tissue in between. That patchy pattern is one of the reasons colonoscopy alone can miss it if the disease is in the small intestine, out of reach of the scope.
Because the inflammation goes through the full thickness of the wall, Crohn's can eventually scar the bowel down to a narrow tunnel (a stricture) or burrow a tunnel from the intestine to another loop of bowel, the bladder, the skin, or the vagina (a fistula). The Core Symptoms
The CDC and the Mayo Clinic Proceedings converge on the same short list. If several of these have been running for weeks, Crohn's belongs on the differential.
Chronic diarrhea
Loose, frequent stools that last more than four weeks. Some patients pass 5 or more bowel movements a day; nocturnal diarrhea - waking up needing to go - is especially suggestive because true IBS almost never wakes people at night. The pattern matters more than the count. A steady drift from Type 4 stools to Type 6 or 7 on the Bristol Stool Chart, for weeks, in someone who has always been regular, is not "just a bug."
Abdominal pain and cramping
Usually in the lower right quadrant when the terminal ileum is involved, which mimics appendicitis. Pain worsens after meals because eating triggers a mass movement of contents into the inflamed segment. Some patients describe a "pulling" or "gripping" pain 30 to 60 minutes after eating, which points at a small-bowel stricture.
Rectal bleeding and blood in stool
Bright red blood on the tissue or in the toilet bowl, or darker blood mixed into the stool. It happens less often in Crohn's than in ulcerative colitis - UC almost always bleeds - but it does happen, especially when Crohn's involves the colon. Unintended weight loss
A loss of about 5% of body weight over three months is one of the more specific warning signs in Crohn's, per AARP's review of the NIDDK criteria. The mechanism is a combination of malabsorption (damaged small-bowel villi cannot pull nutrients through), reduced appetite from ongoing inflammation, and the metabolic cost of chronic disease.
Fatigue
The kind of fatigue Crohn's produces is not "I slept badly." It is the flat, wall-hitting exhaustion of chronic inflammation plus iron-deficiency anemia plus poor absorption of B12 and folate. Roughly half of IBD patients rate fatigue as one of their most disabling symptoms even when other measures of disease activity are quiet.
Fever
Low-grade fever - around 100 to 101 degrees F - can accompany an active flare. Higher spiking fevers suggest an abscess, a complication that changes the treatment plan quickly.
Perianal symptoms
Skin tags, fissures that will not heal, and drainage from small openings around the anus. Perianal disease shows up in about 20 to 30% of Crohn's patients and is essentially unheard of in uncomplicated IBS. It is one of the most Crohn's-specific findings on a physical exam.
Warning Signs That Push a Workup
Any single one of these is a reason to book a gastroenterologist rather than wait it out. Several together is a reason to move quickly.
- Diarrhea lasting more than four weeks, especially if it started without a triggering illness or antibiotic course
- Blood in the stool, particularly if it happens more than once or is mixed into the stool rather than just streaking the tissue
- Waking up at night to have a bowel movement - IBS rarely does this, IBD often does
- Unintended weight loss of roughly 5% or more over three months
- Abdominal pain that keeps recurring in the same spot, especially after meals
- Anemia on routine labs that does not respond to iron replacement
- A perianal fistula, non-healing fissure, or drainage
- A first-degree relative with Crohn's or ulcerative colitis plus any of the above

Extraintestinal Symptoms
Between 25% and 40% of Crohn's patients develop symptoms outside the gut, and those symptoms are frequently what tips a diagnosis. The Johns Hopkins IBD program lists the common ones.
Joints
Peripheral arthritis (large joints - knees, ankles, wrists) that flares with the bowel disease, and axial arthritis (spine and sacroiliac joints) that runs on its own timeline. Enteropathic arthritis is the single most common extraintestinal manifestation of IBD.
Skin
Erythema nodosum (tender red bumps on the shins) and pyogenic gangrenosum (deep, painful ulcers, usually on the legs). Both can appear before the bowel disease is diagnosed.
Eyes
Episcleritis and uveitis - redness, pain, and light sensitivity. Uveitis needs urgent ophthalmology because it can threaten vision.
Liver and bile ducts
Primary sclerosing cholangitis (PSC) is uncommon but strongly associated with IBD, more with ulcerative colitis than Crohn's. Elevated liver enzymes on routine labs in a patient with chronic GI symptoms deserve attention.
Mouth
Recurrent aphthous ulcers (canker sores) show up in a significant minority of Crohn's patients. Cobblestone-appearing oral lesions are less common but essentially diagnostic when biopsied.
Bones
Osteopenia and osteoporosis from steroid use, chronic inflammation, and impaired calcium and vitamin D absorption. Bone density testing is part of long-term IBD care.
Blood
Iron-deficiency anemia from chronic blood loss and malabsorption; B12 deficiency from ileal disease or ileal resection; higher risk of blood clots (deep vein thrombosis, pulmonary embolism), particularly during flares.
Crohn's vs Ulcerative Colitis vs IBS
These three get confused constantly because the symptom overlap is real. The differences that matter clinically:
Crohn's disease - can affect any part of the GI tract, patchy inflammation, goes through the full bowel wall, causes strictures and fistulas, may or may not involve rectal bleeding. Fecal calprotectin is elevated. Endoscopy shows cobblestoning, aphthous ulcers, skip lesions.
Ulcerative colitis - limited to the colon and rectum, continuous inflammation starting at the rectum and moving up, only affects the innermost lining, almost always bleeds, no fistulas or strictures. Fecal calprotectin is elevated. The Mayo Clinic side-by-side is a clean summary.
IBS - functional, not inflammatory. No visible damage on colonoscopy, normal biopsies, normal fecal calprotectin. Symptoms are real (pain, bloating, altered bowel habits) but there is no ongoing tissue injury. IBS is not a "pre-Crohn's" and does not turn into Crohn's; it is a different mechanism. Who Gets Crohn's Disease
Crohn's is not evenly distributed. Some risk factors move the needle a lot.
Family history
The biggest single risk factor. Having a first-degree relative with IBD raises the risk of Crohn's by roughly 5 to 20 times over baseline. Monozygotic twin concordance for Crohn's is around 50 to 55%, one of the highest of any complex disease, which tells you genetics do a lot of the work.
Ashkenazi Jewish ancestry
Prevalence is 2 to 4 times higher in people of Ashkenazi Jewish descent compared with non-Jewish Europeans, a pattern documented across every population study. Cedars-Sinai attributes the difference to founder-effect genetics and shared variants in NOD2, ATG16L1, and other IBD-associated genes.
Smoking
Cigarette smoking roughly doubles the risk of developing Crohn's, worsens flares, raises the need for steroids and immunosuppressants, and increases post-surgical recurrence. (Curiously, smoking is protective against ulcerative colitis, which is one of the strangest findings in IBD epidemiology and does not translate into any reasonable advice.)
Age
Crohn's has a bimodal age distribution. The first and much larger peak is between ages 15 and 35. A second, smaller peak occurs around age 60, and elderly-onset IBD accounts for roughly 10 to 15% of new diagnoses. Kids and older adults get missed more often because the presentation is atypical.
Geography and industrialization
Crohn's is more common in the Northern Hemisphere, in urban areas, and in industrialized countries. Migration studies show second-generation immigrants adopting the risk of their new country, which points at environmental exposures - diet, microbiome, antibiotics - layered on top of the genetic background.
NSAIDs and infections
Heavy NSAID use is associated with flares and possibly with disease onset. Some GI infections appear to trigger IBD in genetically susceptible people, though the causal story is still being worked out.
How Crohn's Disease Is Diagnosed
There is no single test. The diagnosis is put together from bloodwork, stool tests, endoscopy with biopsies, and cross-sectional imaging of the small intestine.
Bloodwork
Complete blood count (looking for anemia and elevated white cells), inflammatory markers (C-reactive protein, ESR), iron studies, B12, folate, vitamin D, and albumin. None are diagnostic by themselves, but a pattern of low iron, low albumin, and elevated CRP in someone with chronic GI symptoms points squarely at IBD.
Fecal calprotectin
A stool test that measures a protein released by neutrophils in the bowel wall. It is the single best test for distinguishing IBD from IBS in primary care. A normal calprotectin has a very high negative predictive value for IBD; an elevated one (typically greater than 150 to 250 mcg/g) is a strong reason to move to endoscopy.
Colonoscopy with ileal intubation and biopsies
The gold-standard test. The endoscopist goes through the entire colon, into the last portion of the ileum, and takes biopsies whether the mucosa looks normal or not. The pathology showing chronic inflammation, granulomas, or transmural changes is what confirms Crohn's. Colonoscopy alone misses disease that is confined to the middle small bowel, which is why imaging is part of the workup.
MR enterography or CT enterography
Cross-sectional imaging of the small intestine after drinking a large volume of oral contrast. MR enterography is the preferred choice because it avoids ionizing radiation, which matters in a young disease with a lifetime of follow-up ahead. Both techniques show bowel-wall thickening, strictures, fistulas, and abscesses that a scope cannot reach. MR enterography is currently considered the best non-ionizing option for evaluating Crohn's.
Capsule endoscopy
A pill-sized camera swallowed to photograph the small bowel. Useful when other tests are inconclusive and there is no suspicion of a stricture (a capsule can get stuck in one).
Complications If Left Untreated
Uncontrolled Crohn's is not something you can wait out. The transmural inflammation drives structural damage over time.
- Strictures - scar tissue narrows the bowel and can cause obstruction. Symptoms include worsening postprandial pain, bloating, and eventually vomiting
- Fistulas - abnormal tunnels between the bowel and another structure, most often another loop of intestine, the bladder, the skin, or the perianal area
- Abscesses - walled-off pockets of infection, often in the abdomen or perianal region, that need drainage and antibiotics
- Malnutrition - iron, B12, vitamin D, and protein deficits that show up on labs long before someone looks visibly malnourished

- Bowel obstruction - a strictured segment can close down completely and become a surgical emergency
- Blood clots - IBD roughly triples the risk of venous thromboembolism, particularly during flares and hospitalizations
Managing Flares
Crohn's runs in flares and remissions. A flare is a stretch of active inflammation with worsening symptoms - more diarrhea, more pain, urgency, blood, fatigue, weight loss, sometimes fever. Recognizing a flare early changes what treatment can accomplish. Harvard Health lists the classic early signs: any change from your personal baseline in stool frequency, urgency, blood, or nocturnal symptoms.
Common flare triggers include NSAIDs, missed medication doses, antibiotics that disrupt the microbiome, gastrointestinal infections, high emotional stress, and cigarette smoking. Diet does not cause Crohn's, but specific foods can worsen symptoms during a flare - typically insoluble fiber, dairy, and high-FODMAP foods. Treatment is stratified by severity. Mild disease may respond to budesonide or aminosalicylates. Moderate to severe disease is treated with biologics - anti-TNF drugs (infliximab, adalimumab), anti-integrins (vedolizumab), anti-IL-12/23 (ustekinumab), or JAK inhibitors and IL-23 selective agents (risankizumab, upadacitinib) that have moved to the front of the line in recent guidelines. Roughly half of Crohn's patients will need surgery at some point, most often to remove a strictured segment or drain an abscess. Surgery is not curative - the disease can recur at the surgical anastomosis - but it can produce years of remission.
What to Track
Symptom tracking is not a nice-to-have in Crohn's. It is the record your gastroenterologist uses to titrate medication, catch flares early, and separate treatment side effects from disease activity. The Crohn's & Colitis Foundation IBD tracker lists the fields that actually matter.
At minimum, log the following daily:
- Bowel movement count and time - including any nighttime episodes
- Stool form on the Bristol scale - looking for a shift toward Type 6 to 7
- Blood in stool - a yes/no is enough, but note the amount if it changes
- Urgency - the difference between "I need to go soon" and "I need to go right now" is clinically meaningful
- Abdominal pain - location and severity, 0 to 10
- Fatigue - same 0 to 10 scale
- Meals and any obvious triggers - food, stress, missed medication
- Medication adherence - biologics work only if you take them on schedule
- Weight, weekly - a downward trend flags a flare before the labs move
Two weeks of that data brought to an appointment beats any amount of "it has been kind of bad lately." It also lets you see, in your own records, whether a new medication is actually working or whether the pattern is unchanged.
The Bottom Line
Crohn's disease is common enough to matter (roughly 1 in 200 U.S. adults), serious enough to require a real workup, and slow-moving enough that patients routinely lose years to a wrong label. If you have had diarrhea for more than a month, blood in the stool, unintended weight loss, or a fatigue you cannot explain, ask for a fecal calprotectin. It is a cheap stool test that answers the "is this IBD or IBS" question with a lot more confidence than a symptom recall.
If you already have a Crohn's diagnosis, the highest-value habit is consistent tracking. Flares caught early are flares treated with a burst of steroids and a medication tweak. Flares caught late are flares treated in the hospital.
