Why Is My Poop Floating? What It Really Means
You glance into the toilet bowl and notice your stool bobbing at the surface instead of sinking. It happens once, then again, then a few times that week. The internet's first instinct is to tell you it's fat malabsorption and to worry about your pancreas. That answer is usually wrong.
The definitive experiment on this question was published in the New England Journal of Medicine in 1972 by Michael Levitt and William Duane. They collected floating and sinking stools, measured their gas and fat content, and did something clever: they compressed the floating stools under pressure to squeeze the gas out. Every single floater sank. What made stool float was trapped intestinal gas, not fat (Levitt and Duane, NEJM 1972). Fifty years later, a 2022 Mayo Clinic study in Scientific Reports nailed it further using germ-free mice: mice with no gut bacteria produced only sinking stool. Colonize the mice with normal gut microbes and their stool starts floating. Gas-producing bacteria - particularly Bacteroides ovatus - are what put the bubbles in there.
So most of the time, a floating stool is a signature of an active, fermenting gut microbiome. The version that actually matters clinically - fat in the stool - has a specific look: pale, greasy, oily, and unusually foul-smelling, most days, for weeks. That's what we'll separate out below.
- Floating stools are almost always caused by trapped gas from bacterial fermentation in the colon, not by fat. This has been settled science since 1972.
- Occasional floaters after high-fiber meals, beans, dairy, or artificial sweeteners are normal and mean your microbiome is doing its job.
- Fat malabsorption stool (steatorrhea) is a different pattern: pale, greasy, oily, sticks to the bowl, unusually foul, and happens most days.
- Persistent greasy floaters warrant a workup for celiac disease (~1% of adults), exocrine pancreatic insufficiency, chronic pancreatitis, bile acid problems, and Giardia infection.
- Red flags that turn a curiosity into a doctor's visit: unintentional weight loss, chronic diarrhea over 4 weeks, oily film in the bowl, deficiency symptoms, or floaters plus abdominal pain.
- A stool tracker beats memory here. The distinction that matters is pattern over weeks, not one specific bowel movement.
The Physics: Why Anything Floats or Sinks in Water
A stool floats if its density is less than water's. Density is mass divided by volume. To make a stool less dense, you either subtract mass (which nothing in your GI tract really does) or add volume without adding mass. The one thing that inflates volume without adding meaningful mass is gas. That's the entire mechanism. A stool with pockets of hydrogen, methane, and carbon dioxide bubbled through it becomes a lighter-than-water composite object and floats.
Fat is a distant second. Fat is less dense than water (which is why oil floats on top of vinegar in salad dressing), so a stool loaded with unabsorbed fat can float too. But fat has to be present in high quantities to shift the average density enough, and that only happens in true malabsorption. Levitt and Duane's 1972 measurements found no meaningful difference in fat content between floaters and sinkers in healthy subjects. The difference was gas.
The Landmark 1972 Study Nobody Cites Correctly
The Levitt and Duane paper compared 33 healthy volunteers (9 with floating stools, 24 with sinking) against 6 patients with confirmed steatorrhea. Two things came out of it:
- Fat content did not distinguish floaters from sinkers in healthy people. The floaters had no more fat than the sinkers. Both groups were within the normal range.
- Gas did. When floating stools were placed in a chamber and pressurized, the gas dissolved into the stool matrix, the volume shrank, the density rose, and the stool sank. Every time. The buoyancy was mechanical - a gas problem, not a chemistry one.
The healthy floaters in that study also had higher rates of methane production, which is genetically variable. Roughly a third of adults host methanogens (like Methanobrevibacter smithii) in numbers large enough to detectable methane in a breath test. Those people are structurally more likely to produce floaters even on a normal diet. It's a trait, not a disease.
The 2022 germ-free mouse experiment tightened this even further. Germ-free mice (raised in sterile isolators so no bacteria ever colonize their gut) produced only sinking pellets. When the researchers colonized those same mice with normal gut flora, their stool started to float within days. The team identified over ten gasogenic bacterial species enriched in the floaters, with Bacteroides ovatus the most prominent. That's causal proof: microbes make gas, gas makes floaters.
When Floating Is Normal (Most of the Time)
Occasional floaters are boring news. They usually track back to something you ate that fed extra fermentation in the colon over the next 12-24 hours.
Common benign triggers
- High-fiber meals. Beans, lentils, chickpeas, broccoli, Brussels sprouts, cauliflower, onions, garlic, cabbage. All contain fermentable carbohydrates (FODMAPs, resistant starch, or oligosaccharides) that pass to the colon intact and feed bacteria that produce hydrogen and methane. - Dairy in the lactose-intolerant. Undigested lactose reaches the colon, bacteria have a field day, hydrogen production spikes, stool floats. About 68% of the world's adult population has some degree of lactase non-persistence.
- Sugar alcohols and artificial sweeteners. Sorbitol, xylitol, mannitol, erythritol - the "-ol" sugars in sugar-free gum, protein bars, and diet drinks. Poorly absorbed by design, aggressively fermented in the colon.
- A cold or GI bug. Any acute infection that transiently disturbs digestion can throw off your normal gas balance for a few days.
- Antibiotics or a new probiotic. Rearranging the microbial community shifts which gases dominate. Temporary floaters after starting or stopping a course are expected.
- You're just a methane producer. Roughly one in three adults hosts methanogens that produce methane during fermentation, which correlates with more buoyant stool independent of diet.
If your floaters follow a bean chili on Sunday, disappear by Wednesday, and don't come with other symptoms, you have your answer. Move on.
When Floating Is a Problem: The Steatorrhea Pattern
Steatorrhea - fat in the stool - is what gets people worried, and correctly so. It's the malabsorption signature. But it doesn't look like an ordinary floater. It has a specific look, smell, and behavior that's hard to mistake once you know what to watch for.
What steatorrhea actually looks like
- Pale. Yellow, tan, clay-colored, or grey. The color of fat combined with reduced bile pigment.
- Greasy or oily. Visibly shiny. May leave an oil slick or droplets in the toilet water.
- Sticky. Coats the bowl. Hard to flush. Requires more toilet paper than makes sense.
- Voluminous and loose. Larger volume than normal, often loose or fully liquid.
- Foul in a distinct way. Not just "bad" - rancid, sharp, penetrating. Different from ordinary bad-smelling stool. See our smell writeup for how malabsorption smell differs from a sulfur pattern.
- Persistent. Most days, over weeks. Not one bowel movement after a fried meal.
The clinical definition of steatorrhea, per Domínguez-Muñoz and colleagues (2017), is fecal fat excretion greater than 7 grams per day while eating 100 grams of fat per day. That's measured with a 72-hour fecal fat collection, which almost no one does anymore because fecal elastase-1 (a single stool sample testing pancreatic enzyme output) has largely replaced it. But the pattern - pale, greasy, sticky, foul, frequent - is the clinical picture you'll present with regardless of the test used.
Conditions Behind Persistent Fatty Floaters
If the pattern above is what you're seeing, a handful of conditions cover almost all the diagnostic ground. They all interfere with one of the three steps needed to absorb fat: bile from the liver to emulsify it, pancreatic lipase to break it down, and a healthy small intestinal lining to absorb it. Break any of those and fat ends up in the toilet.
Celiac disease
Celiac disease damages the small intestinal villi via an autoimmune response to gluten, and the flattened villi can't absorb fat (or much of anything else) properly. Prevalence in the United States is roughly 0.71% of the population - about 1 in 141 people - based on the Rubio-Tapia et al. NHANES analysis, and closer to 1% in non-Hispanic whites. The NIDDK puts the total at about 2 million Americans, most of them undiagnosed. Screening is simple: a tissue transglutaminase IgA blood test with total IgA, before you've cut gluten out. If you've already gone gluten-free, the test can be falsely negative.
Exocrine pancreatic insufficiency (EPI) and chronic pancreatitis
Your pancreas makes lipase, the enzyme that digests fat. Damage the pancreas enough and lipase output drops until fat starts slipping through undigested. Chronic pancreatitis is the leading cause, followed by cystic fibrosis, pancreatic surgery, and pancreatic cancer. EPI is diagnosed in 84.6% of chronic pancreatitis patients based on symptoms of bloating, steatorrhea, or weight loss (Capurso et al., 2019), and it also shows up in about 34% of patients at 12 months after a single episode of acute pancreatitis (Phillips et al., 2024). Testing is typically fecal elastase-1 in a single stool sample; results below 200 μg/g suggest EPI. Treatment is pancreatic enzyme replacement therapy with meals.
Bile acid problems
Bile from the liver emulsifies fat so lipase can work on it. If bile isn't reaching the small intestine (blocked bile duct, gallbladder issues, liver disease) or isn't being recycled properly (bile acid malabsorption), fat digestion breaks down. Post-cholecystectomy bile acid diarrhea is a common under-recognized version: a 2018 meta-analysis found that 70% of patients with chronic watery diarrhea after gallbladder removal had bile acid diarrhea confirmed by SeHCAT testing. Response to bile acid sequestrants (cholestyramine, colesevelam) is 79%, which makes it a highly treatable diagnosis - if anyone thinks to look for it.
Giardia and other parasitic infections
Giardiasis is the most common parasitic GI infection in the US, per the CDC. It's classically waterborne (contaminated drinking water, backcountry streams, daycare exposures). Giardia parks itself on the small intestinal lining and impairs fat absorption, producing weeks of pale, greasy, floating, foul stools along with bloating, cramping, and sometimes weight loss. A stool antigen test is the standard workup; treatment is typically metronidazole or tinidazole.
Small intestinal bacterial overgrowth (SIBO)
Overgrown bacteria in the small intestine can deconjugate bile acids and interfere with fat absorption, producing steatorrhea in more advanced cases. Inflammatory bowel disease and short bowel syndrome
Crohn's disease affecting the terminal ileum impairs bile acid reabsorption and fat digestion. Extensive small bowel resection (short bowel syndrome) leaves too little surface area to absorb fat. Both produce steatorrhea proportional to how much of the small intestine is compromised.
Cystic fibrosis
In children and young adults, CF is one of the more common causes of pancreatic insufficiency and its characteristic fatty stools. Almost always diagnosed in childhood via newborn screening, but occasionally milder cases turn up in adults.
The Two-Column Test: Gas Floaters vs Fat Floaters
Here's the practical distinction. Most people can sort themselves cleanly with this.
Probably gas (benign)
- Stool color is normal brown
- Doesn't look greasy or oily
- Flushes without residue or streaks
- Happens occasionally, often after a specific meal (beans, dairy, fiber load, sugar alcohols)
- Not accompanied by weight loss, chronic diarrhea, or nutritional issues
- Bristol type is normal for you (see the Bristol Stool Chart guide for the reference)
Probably fat (worth a workup)
- Pale, yellow, tan, or grey color
- Visibly oily or greasy, sometimes with a film on the water
- Sticks to the bowl, difficult to flush
- Distinctly foul, rancid smell
- Happens most days for two or more weeks
- Comes with weight loss, chronic diarrhea, fatigue, or signs of vitamin deficiency (bruising, bone pain, night blindness)
If your stools cluster in the right column, book an appointment with a gastroenterologist. If they cluster in the left, tracking what you ate in the 24 hours before will usually solve the mystery within a week.
Other Things That Change Stool Buoyancy
A few contributors don't fit neatly in the gas-versus-fat frame but are worth knowing about.
- Antibiotics. Wiping out normal gut flora shifts which bacteria dominate and which gases they produce. Some people become temporary floaters during and after a course.
- Rapid transit. When stool moves through the colon fast (viral gastroenteritis, IBS-D, laxatives, coffee-driven urgency), less water is reabsorbed, the stool is looser, and pockets of gas get trapped more easily. Related: our coffee and pooping writeup.
- Air swallowing (aerophagia). Rare, but people who swallow large volumes of air (fast eating, carbonated drinks, chronic gum chewing, anxiety) can pass more gas through the GI tract, some of it in the stool.
- Colorectal transit time. The longer stool sits in the colon, the more fermentation happens and the more gas ends up in the final product. This is one reason people with slow transit and constipation sometimes report floating stools once things finally move.
When Floaters Are an Emergency
Floating stool alone is not an emergency, ever. What makes it urgent is what comes with it.
Go to urgent care or the ER for:
- Severe abdominal pain, especially in the upper abdomen or radiating to the back (possible acute pancreatitis)
- Fever with jaundice (yellowing of skin or eyes) and pale stools (possible bile duct obstruction)
- Vomiting blood or passing black tarry stools
- Rapid dehydration from ongoing diarrhea
Book a gastroenterologist within a few weeks for:
- Floating stools that are also pale, greasy, and foul, happening most days for more than two weeks
- Unintentional weight loss of more than 5% of body weight over a few months
- Chronic diarrhea over four weeks
- Symptoms of malabsorption: fatigue, easy bruising, bone pain, tingling in hands and feet
- New floaters plus a personal or family history of celiac disease, IBD, or pancreatic disease

What Your Doctor Will Do
If you show up with a persistent steatorrhea pattern, the workup is fairly standardized. Knowing what to expect helps you push for what's useful and skip what isn't.
- Bloodwork: complete blood count, comprehensive metabolic panel, liver enzymes, vitamin B12, iron studies, vitamin D, and fat-soluble vitamin levels (A, E, K) if malabsorption is likely. Celiac serology (tissue transglutaminase IgA plus total IgA) is standard.
- Fecal elastase-1: a single stool sample that measures pancreatic enzyme output. Under 200 μg/g suggests exocrine pancreatic insufficiency; under 100 is more definitive.
- Stool studies: Giardia antigen, C. difficile toxin if applicable, occult blood, and sometimes a qualitative fecal fat (Sudan stain) or the older 72-hour quantitative fecal fat collection.
- Imaging: abdominal ultrasound or CT of the pancreas if EPI or chronic pancreatitis is suspected.
- Endoscopy with small bowel biopsy: the diagnostic gold standard for celiac disease and a way to rule out other small bowel pathology.
- Breath testing: hydrogen and methane breath tests for SIBO if the picture fits.
Not every workup includes every test. A good gastroenterologist reads the pattern first and orders tests targeted at the most likely explanation, not a shotgun panel.
The Tracking Argument
Floating stool is one of those symptoms that's almost useless in isolation and highly useful in pattern. One floater on a Tuesday tells no one anything. Fifteen floaters over three weeks, most of them pale and greasy, most of them after specific meals, alongside a five-pound weight loss - that's a story your gastroenterologist can work from.
The two things worth tracking are what the stool looks like (color, buoyancy, greasiness, Bristol type) and what came in the day before. Food logging alongside stool logging is how people identify the sugar alcohol in their new protein bars, the lactose in the "just a little cheese," or the pancreatic insufficiency they've been shrugging off for a year. If you're seeing enough floaters to be reading a 2,000-word article about it, you have the volume of data to make a real judgment. Log it in something structured for two weeks. If the pattern points at gas, you'll almost certainly be able to name the food triggers. If it points at fat, you'll walk into your appointment with a record that shortens the diagnostic loop by weeks.
